If you wish to join a Crush Speed-Dating Event, please complete the following Sign-Up Form and our staff will contact you shortly for confirmation. All associated fees are collected during the event.

** Referral Program : By submitting a Joint Application with a referral of a new client, you will be entitled to special discounts on the admission fee for the Crush Speed-Dating Event. Simply include the name(s) of the Referral(s) in the Application Form and attendance at the captioned event, ALL APPLICANTS will receive a reduction of their admission fee as stated below:

One - Two Referrals : $10.00
Three or More Referrals : $20.00
